Rheumatic diseases are chronic conditions which affect a large proportion of the population worldwide . Among them, inflammatory chronic diseases including rheumatoid arthritis , spondyloarthritis , crystal-induced arthritis , and connective tissue autoimmune diseases, such as systemic lupus erythematosus and systemic sclerosis , are the most common. Chronic inflammation has been recognized to play a role also in osteoarthritis , classically considered a non-inflammatory form of arthritis.

Although different risk factors influence their onset and development, these heterogeneous conditions share common clinical outcome including joint pain, disability, and comorbidities .

The most commonly used are Lactobacilli , Bifidobacteria , and Streptococci , either as single species or in mixed culture. Their health effects are largely recognized and supported by randomized controlled clinical trials and meta-analyses conducted on infectious diseases, antibiotic-associated diarrhea, irritable bowel syndrome, abdominal pain, and colitis .

Preparation Of Lactobacillus Gasseri Pa3 Supplements

Lactobacillus gasseri PA3 with an ability of purine absorption was isolated from Lactobacillus gasseri PA3 drinking yogurt by the method of Jang et al. and cultured in Man Rogosa Sharpe liquid medium at 37°C for 48 hr. MRS : peptone 10.0, beef extract 5.0, glucose 20.0, Tween 80 1.0, dipotassium phosphate 2.0, sodium acetate 5.0, triammonium citrate 2.0, magnesium sulfate 0.2, manganese sulfate 0.05, and agar powder 14.5. After 15 min of centrifugation at 6,000g, supernatant was discarded and isometric fermented nutritive medium was added to suspend the cells. FNM is composed of: peptone 3.0, corn starch 8.0, yeast extract 4.5, tryptone 3.0, Mucin 0.5, l-cysteine hydrochloride 0.8, bile No.3 0.4, heme 0.05, sodium chloride 4.5, Tween 80 1.0, potassium chloride 2.5, potassium dihydrogen phosphate 0.4, magnesium chloride hexahydrate 4.5, calcium chloride hexahydrate 0.2, and 2 ml of trace elements stock solution .

Metagenomic Analysis Shows That The Gut Microbiome May Have A Role In Gout

Compared with healthy individuals, patients with gout were found to have a significantly unique gut microbiota associated with dysregulated host urate degradation and systemic inflammation, according to study results published in NPJ Biofilms and Microbiomes.

Given the growing evidence suggesting a link between the gut microbiome and arthritis, Chu and colleagues sought to investigate the taxonomic and functional signatures of the gout microbiome, its association with gout-related clinical parameters, response to therapeutic interventions, and identification of gout-specific microbiome signatures compared with the other autoimmune and metabolic disorders.

In the current large-scale metagenomic analysis, the researchers examined the gut microbiome of 307 fecal samples collected longitudinally from 102 patients with gout and 86 healthy individuals. The study group included 2 cohorts the discovery cohort included 140 samples at baseline and 70, 40, and 9 fecal samples collected longitudinally from patients with gout at week 2, 4, and 24 post-baseline, respectively the validation cohort included 48 samples .

Using a random forest model, the researchers constructed a disease classifier based on the microbial gene-level profile. Three genes were significantly enriched in patients with gout in the discovery cohort , and 2 of the genes showed an increasing trend in the validation cohort .

    How Can Gout Be Managed

    If you do develop gout in a joint, rest it and ice it. You can take Ibuprofen to manage the pain and reduce inflammation but unless you need to take Aspirin for another health problem, you should avoid it, as it can actually increase uric acid levels and make the gout worse.

    If you do need to take Aspirin while suffering with gout, consult your doctor about appropriate doses and the best ways to manage the problem. Advice from the Gout Society, is that low doses when prescribed for prevention of heart attacks should not increase your uric acid levels significantly and should be continued.

    Overview Of Gut Metagenome In Gout Patients And Controls

    High Potency Probiotic

    We performed metagenomic shotgun sequencing for 307 fecal samples collected from 188 individuals. At baseline, there were 140 samples in discovery cohort collected during 20162017, including 77 gout patients and 63 healthy controls. There were 48 samples in validation cohort collected in the year of 2018, consisting of 25 gout patients and 23 healthy controls. In addition, 70, 40, and 9 fecal samples were collected from gout patients in discovery cohort longitudinally at three follow-up time points , to assess the effects of therapeutic intervention on the gut microbiome in gout patients. All patients information is summarized in Supplementary Data . Compared with controls, gout patients had elevated levels of inflammation and renal indices, including C-reactive protein , erythrocyte sedimentation rate , SUA, and serum creatinine . There was a higher body mass index in gout patients, in accordance with the view that gout was associated with obesity. In addition, CRP, ESR, SUA, and other gout-associated indices including visual analog scale , arthralgia, joint swelling scores and joint tenderness scores declined in gout patients following treatment .
