Lactobacillus Rhamnosus Gg And Cystic Fibrosis

Individuals with cystic fibrosis often display high levels of inflammation, mucus formation and microbial dysbiosis . There is potential for the use of probiotics, especially given their ability to reduce RTIs through the gut-lung axis. L. rhamnosus GG has been assessed in a few pilot studies in children with CF.

A controlled prospective study specifically assessed intestinal inflammation in 30 children with CF. A total of 75 children were included 30 with CF, 30 healthy controls and 15 with active inflammatory bowel disease. Ten of the children with CF received 5 billion L. rhamnosus GG daily for four weeks. After intervention faecal calprotectin levels significantly decreased in this group. These results highlight the potential of L. rhamnosus GG to reduce gut inflammation in CF patients . Reduced calprotectin levels were also found in a later trial using L. rhamnosus GG, the study also found distinct microbial changes in CF individuals .

Surprisingly a larger recent study in 2018, found L. rhamnosus GG did not have beneficial effects in children with CF. The RCT divided 95 children to receive either 6 billion CFU of L. rhamnosus GG or a placebo daily for 12 months. No differences in pulmonary exacerbations or hospital admissions were noted. The authors concluded that perhaps the trial was conducted under more stringent conditions or whether a childs age may play a role in L. rhamnosus GG efficacy .

An extracellular polysaccharide layer is commonly found in lactobacilli. Of interest, EPS molecules show a large structural diversity , so that they are clearly strain-specific molecules. The LGG cell surface appears to contain two major types of polysaccharides: long galactose-rich polysaccharides and shorter glucose/mannose-rich polysaccharides . We could yet identify the operon responsible for galactose-rich EPS synthesis . A knock-out mutant of the welE gene, encoding the priming glycosyltransferase, is completely devoid of the long galactose-rich EPS but shows a higher concentration of short glucose-rich EPS . This mutant shows an increased adhesion to Caco-2 IECs, linked to increased exposure of the SpaCBA pili . In addition, this type of galactose-rich EPS appears to be an important adaptation factor for LGG as the welE mutant shows a reduced in vivo survival in the murine gastrointestinal tract . More specifically, the long galactose-rich EPS were shown to protect the cell against complement-mediated lysis and cathelicidins, specific cationic antimicrobial peptides . It thus seems important that this type of EPS production is balanced between optimal protection and optimal adhesion.

Probiotics contain microorganisms, most of which are bacteria similar to the beneficial bacteria that occur naturally in the human gut. They are available over-the-counter or by prescription and in a variety of forms such as capsules, packets, or food supplements. Although most probiotics are available without a prescription, there may be an advantage to patients with prescription drug coverage because probiotics may be a covered benefit. Probiotics have been widely studied in a variety of gastrointestinal diseases, and one in five Americans takes probiotics for digestive problems.1 The most studied probiotics for human use belong to the Lactobacillus, Bifidobacterium, or Saccharomyces species.2 This article focuses on probiotic use in infants, children, and adults with GI conditions, and it excludes probiotics for non-GI diseases.

Study Eligibility For Efficacy

Inclusion criteria for articles assessing probiotic efficacy included: randomized controlled trials of a probiotic intervention , in adults or children, reported duration of intervention and/or follow-up, for either prevention or treatment of disease and peer-reviewed publication . We included RCTs with primary outcomes of either prevention or treatment of various diseases and, if present, secondary outcomes and additional treatment arms in this review. As the efficacy of probiotics is both strain-specific and disease-specific , each type of probiotic was required to have at least two RCTs for each disease indication being considered. The probiotic intervention was required to be well defined by strain or strains if a mixture of species were used and the daily dose. Exclusion criteria for efficacy assessments included: reviews, kinetic or safety studies, non-randomized trials, duplicate reports, and trials with insufficient descriptions of the type of probiotic. Probiotic products with less than two RCTs per disease indication were excluded. Disease indications with less than two RCTs per probiotic type were also excluded. Pre-clinical studies of mechanisms of action or safety studies were also excluded from the efficacy assessments.

Twenty-five types of probiotic products commonly found as over-the-counter dietary supplements or on-line are shown in . Unfortunately, the health claims on the labels of dietary supplements often do not provide practical clinical guidance for healthcare providers nor for the general public. Several probiotic products are being developed as potential drugs under the F.D.A. regulated pathway, but none have been approved currently as prescription products. Probiotics have a long history of use in Europe and Asia, but in 2013, the use of probiotic was disallowed on the label due to the disparity of evidence for various claims for health benefits . Currently, regulations relating to probiotics are in flux and differ dramatically from country to country.

Antistress Effects Of Probiotics

Probiotic Strain Lactobacillus Rhamnosus and the Upper Respiratory Tract

Barreau et al. have reported that probiotic treatment consisting of Lactobacillus rhamnosus and Lactobacillus helveticus can ameliorate enhanced basal HPA axis activity induced by maternal separation stress in rats, suggesting that probiotics normalize the activity of the HPA axis. The same treatment with probiotics also ameliorated water avoidance stress-induced elevations in serum corticosterone levels compared with placebo . In line with these observations, Bravo et al. showed that the levels of stress-induced corticosterone are significantly lower in stressed mice fed L. rhamnosus compared with their control broth-fed counterparts. Thus these findings support the notion that probiotics exert a protective effect on stressed hosts.

Secreted Proteins As Probiotic Effectors

In 2002, Yan and Polk showed that LGG promotes the survival of intestinal epithelial cells by preventing cytokine-induced apoptosis through blocking of p38 MAP kinase. They found that the survival-promoting effect was also present in other probiotic strains such as L. acidophilus ATCC393 and L. casei ATCC4356, but the strongest in LGG . The supernatant of LGG was shown to prevent apoptosis in IECs and induce heat shock proteins . Consequently, two proteins from the LGG supernatans were found to cause the antiapoptic effect . These proteins p75 and p40 were later renamed as Major Secreted Protein Msp1 and Msp2 respectively, because of the discrepancy in molecular weight . Each of the purified proteins was shown to activate the Akt signaling peptide, inhibit cytokine-induced IEC apoptosis and reduce tumor necrosis factor – induced epithelial damage. The proteins also promoted cell growth in human and mouse colon IECs and cultured mouse colon explants . Moreover, they were shown to protect the intestinal epithelial barrier function from hydrogen peroxide-induced damage through blocking of MAP kinases .
