Probiotics And Vaginal Infections

Probiotics for Women’s Health – Chapter 12

Probiotics are microorganisms that produce beneficial effects on human health, such as preventing vaginal infections, maintaining optimal pH balance, supporting immune function, and preventing harmful microbes from thriving.

Scientists have identified two species of probiotic bacteria with remarkable benefits for female health. Known as Lactobacillus rhamnosus and Lactobacillus reuteri , these probiotics show powerful effects in preventing and fighting bacterial and fungal vaginal infections.

Bacteria: Friends Or Foes

We often think of bacteria as the enemy: Its certainly true that many species such as those that cause tuberculosis or anthrax are capable of great harm. But we would be remiss to ignore the fact that some bacteria, particularly those that produce lactic acid, are not only harmless, theyre actually highly beneficial. In fact, a beneficial symbiotic relationship exists between humans and certain types of friendly bacteria.

Although we enter the world with sterile bodies, the situation changes rapidly. Beneficial bacteria lose little time colonizing areas where body tissues interface with the environment, such as the mouth, digestive tract, and vagina. When beneficial bacteria hold sway, optimal health is achieved and maintained. In the gut, friendly bacteria help maintain the integrity of the intestinal lining and emerging evidence suggests they also boost immunity.1-4

Happily, by flourishing they also prevent other, far less innocuous bacteria from gaining a foothold, both by physically crowding them out, and by optimally altering the local environment to discourage harmful bacteria from thriving. An individuals beneficial microflora eventually number into trillions of individual cells. In the gut, these friendly microorganisms contribute to the bodys supplies of vitamin K and folic acid5,6by interacting with our own cells in a highly complex but harmonious relationship,7 as well as helping modulate innate immunity.2

Evaluation Of The Key Species

Figure 5 The abundance of the top abundant bacterial species and key Lactobacillus species in vaginal microbiota. The relative abundances of bacterial species with abundance above 0.01 in the vaginal environment of BV patients have been evaluated . The relative abundance of vaginal dominant Lactobacillus species , including L. crispatus, L. gasseri and L. iners, as well as the intervention probiotic species in the vaginal microbiota have also been explained. Wilcoxon test was used to do the statistical analysis of each two independent groups, and p-value was noted by stars above the bars. *stands for p-value < 0.05 **stands for p-value < 0.01 ***stands for p-value < 0.001.

Boric Acid Suppositories For Bv

Boric acid suppositories have long been used for vaginal infections, including BV and yeast infections. It is regarded as a simple, safe home remedy for vaginal infections. It is thought that boric acid may affect the biofilms of BV-causing bacterial.

One study showed that treatment with nitroimidazole followed by 21 days of intravaginal boric acid suppositories and then, if in remission, metronidazole gel twice weekly for 16 weeks. BV cure rates at 12, 16, 28, and 36 weeks were 87%, 78%, 65%, and 50%, respectively.

Though the BV cure was not permanent for 50% of women in the trial, this study indicates that boric acid , may be useful in delaying recurrence of BV. More studies are needed to determine if ongoing use of boric acid may be helpful for BV prevention, especially for those with recurring BV.

Please note: Oral use of boric acid is toxic vaginal use only. Pregnant women should not use boric acid.

You can purchase boric acid suppositories online quite easily. As always, it is best to discuss any of these remedies with your doctor before use.

Other home remedies for BV that are less studied include: herbal suppositories , tea tree oil for BV, and hydrogen peroxide for BV.

If you are considering any BV treatment over the counter or home remedies for BV, talk to your doctor.

Home Treatment Of Bacterial Vaginosis That Works

Probiotics can be an effective home treatment of Bacterial Vaginosis for many women. But a home remedy won’t work unless you chose the right probiotic.

And the right one is any supplement that contains Lactobacillus rhamnosus GR-1 and Lactobacillus reuteri RC-14 in the correct amount.

My personal favorite probiotic supplement for these two is Fem Dophilus because it contains those two probiotics in the same strength that was used in the successful studies.

If you’re looking for an effective home treatment of Bacterial Vaginosis it makes sense to duplicate what was done in the trials.

Even the Mayo Clinic suggests probiotics for BV!

So – big drumroll, please – introducing

  • Lactobacillus reuteri RC-14

Research has shown that some Lactobacilli need to be inserted vaginally whilst others can be taken as an oral supplement and yet still be effective in the vaginal/urethal area.

Lactobacillus rhamnosus GR-1 and Lactobacillus fermentum RC-14 have both proved particularly good at this “ascent from the rectum after oral ingestion” as the scientists like to put it which makes them very suitable for home treatment of bacterial vaginosis.1,2,3

“The ingestion of the GR-1/RC-14 probiotic therapy resulted in the vaginal flora being restored to normal within 28 days in 82% in one group of patients.”

Remember, these ladies were taking their probiotic supplement for BV by MOUTH, not inserting it vaginally!

Microbial Adhesion To Hydrocarbons

The cell surface hydrophobicities of the Lact. rhamnosus GR-1 and Lact. reuteri RC-14 were determined by a MATH test according to Kos et al. with a slight modification. Xylene and toluene were used as the hydrocarbon solvents in this test. Overnight cultures of Lact. rhamnosus GR-1 and Lact. reuteri RC-14 strains were prepared as described for the autoaggregation and coaggregation assays . One millilitre of xylene or toluene was added to 3 ml of each lactobacilli cell suspension, and the solutions were incubated for 10 min at room temperature. Subsequently, the two-phase solutions were vortexed for 2 min and incubated for an additional of 20 min at room temperature. The hydrocarbon was removed completely, and the absorbance of aqueous-phase cell suspension was measured at 600 nm. The percentage of cell surface hydrophobicity was expressed as 100 × where, ODA is the absorbance after mixing with hydrocarbon solvents and ODB is the absorbance before mixing with hydrocarbon solvents.

What Is Lactobacillus Rhamnosus

L. rhamnosus is a type of bacteria found in your intestines.

It belongs to the genus Lactobacillus, a type of bacteria that produce the enzyme lactase. This enzyme breaks down the sugar lactose which is found in dairy into lactic acid.

Bacteria from this genus, such as L. rhamnosus, are considered probiotic.

Probiotics are live microorganisms that can offer health benefits when consumed .

Hundreds of studies support the benefits of L. rhamnosus.

Uniquely adapted to survive in acidic and basic conditions within your body, this bacterium can also adhere to and colonize your intestinal walls. Such characteristics give L. rhamnosus a better chance of survival so it may offer longer-term benefits (

However, many products that contain L. rhamnosus dont typically include it in the ingredients list.

Summary

L. rhamnosus is a member of the probiotic Lactobacillus genus of bacteria. It is adapted to survive in your gut, thus potentially offering longer-term benefits.

L. rhamnosus has numerous potential benefits and uses for your digestive system, as well as other areas of health.

Bacterial Vaginosis Vs Yeast Infection

Despite the fact that yeast infections are talked about much more, they are the second most common cause of vaginal symptoms, beat out only by bacterial vaginosis.

So whats the difference between the symptoms of bacterial vaginosis and a yeast infection?

Common symptoms of a yeast infection include itchiness or burning in or around the vagina, and a white discharge that often looks like cottage cheese. This discharge is typically odorless and might have a yeasty smell .

This differs from the thin consistency and strong fishy odor of the discharge associated with bacterial vaginosis.

Of course, the best thing to do if you have any of these symptoms is to visit your healthcare provider who can perform an exam and testing to determine which of these infections you might have.

Vaginal And Faecal Microbiota Sample Processing And Analysis

At enrolment and each follow-up time point, two vaginal samples were collected by specimen collection swabs Technology Co., Ltd., Guangdong, China): one for the screening of BV by Nugent scoring and to measure vaginal pH, and the other was stored in 3 ml of buffer Technology Co., Ltd, Guangdong, China) at -80°C and later used for vaginal microbiota DNA profiling. All the patients were asked to self-collect a sample of faeces at enrolment and each follow-up visit using a self-collection bag containing the transport medium from BGI. The faecal samples were also stored at -80°C immediately upon receipt at the laboratory.

Total DNA of vaginal and faecal samples was extracted using a QIAamp DNA Mini Kit and DNA concertation was determined by a Qubit Fluorometer, and the purity and integrity of the gDNA were measured by 0.6% agarose gel electrophoresis. The DNA samples were stored at -80°C for further usage. Qualified genomic DNA samples were configured with the specific primers 8F and 518R , and the variable regions V1 to V3 of the 16S rRNA gene were amplified by PCR. Subsequently, Agencourt AMPure XP magnetic beads were used to purify PCR amplicons, which were dissolved in TE buffer and labelled to complete library construction. An Agilent 2100 Bioanalyzer was used to detect the insert fragment size, and then the products were sequenced on the HiSeq2500 platform using the PE300 module.

The candidacidal activities of Lact. rhamnosus GR-1 and Lact. reuteri RC-14 against the C. glabrata strains were evaluated by CLSM and by using a commercial LIVE/DEAD® yeast viability kit following manufacturer’s instructions. Briefly, overnight cultures of lactobacilli and C. glabrata strains were diluted to OD600 nm values of 0·5 and 1·0, respectively. Equal volumes of lactobacilli and C. glabrata strains were mixed and co-incubated for 24 h at 37°C. For a nontreated control, C. glabrata strains were grown in MRS broth without the inclusion of lactobacilli strains. Cells were washed with GH solution , mixed with 12·5 mol l1 FUN-1 cell stain and incubated in the dark for 30 min at 30°C. Subsequently, the cell suspensions were combined with Calcofluor White M2R to a final concentration of 25 mol l1 and incubated at 30°C in the dark for an additional 10 min. The metabolic activities of the C. glabrata cells were observed using a FluoView FV1000 confocal laser scanning microscope using multipass filter sets appropriate for viewing 4,6-diamidino-2-phenylindole , fluorescein and rhodamine . Images were produced using the fv10-asw viewer software, ver. 4.0 .

Vaginal Microflora Disorders Exceptionally Common

According to one report, there are more than 300 million cases of urinary tract infections, bacterial vaginosis, and yeast vaginitis worldwide every year.10 Yeast infections alone affect 75% of American women during their reproductive years, and 40-50% of these women will endure recurrent episodes, while 5-8% experience chronic Candida infections.11

Standard treatment for the common Candida yeast infection calls for antifungal therapy, either with an oral agent, such as itraconazole or fluconazole,12 or a more cumbersome vaginal cream or suppository, which may require multiple applications. Bacterial vaginosis is commonly treated with an antibiotic, such as metronidazole or clindamycin.13 In any event, scientists note that recurrent infections of this type are notoriously difficult to manage, and can cause a great deal of discomfort, inconvenience, and even psychological distress.14

Given the high prevalence of these infections, and the alarmingly high rate of recurrence, it is clear that most adult women could benefit from preventive protection.

Study Groups And Randomization

Probiotics for Women’s Health – Chapter 10

A total of 328 women between 12- and 16 weeks of gestation consented and were screened between May and October for the presence of an intermediate or high Nugent score . Eighty-six women whose vaginal samples had a Nugent score ⥠4 were randomized.

Consort flow chart of pregnant women enrolled in the study.

A Z test was used to determine the sample size with alpha = 0.05 and power = 0.8, giving a per-group sample size of 40 women to detect a difference between an asymptomatic Intermediate/BV prevalence of 30% in the probiotic group and 60% in the placebo group at the end of treatment protocol. This was increased to 43 in each group to compensate for 5% lost to follow-up. Subjects were randomized using a web-based service to receive two identical looking capsules per day containing either GR-1 and RC-14 or placebo given orally for 12 weeks. Vaginal swabs were collected at 13-, 28- and 35 weeks of gestation and analyzed for Nugent score, cytokines, chemokines and microbiota. Fourteen subjects were lost to follow-up or withdrew, 3 had taken less than 25% of the capsules, and there was insufficient sample for analysis in 3 others. This left 32 subjects in the probiotic and 34 in the placebo group with samples available for sequencing analysis and samples from 31 probiotic and 33 placebo subjects for cytokines and chemokine analysis.

Clinical Study Comparing Probiotic Lactobacillus Gr

Microbes Infect. 2006 Oct 8:2772-6. Epub 2006 Sep 11.

Clinical study comparing probiotic Lactobacillus GR-1 and RC-14 with metronidazole vaginal gel to treat symptomatic bacterial vaginosis.

Anukam KC, Osazuwa E, Osemene GI, Ehigiagbe F, Bruce AW, Reid G. Department of Pharmaceutical Microbiology, Faculty of Pharmacy, University of Benin, Nigeria.

Bacterial vaginosis is particularly common in black women, and in Nigeria it is often caused by Mycoplasma, as well as Atopobium, Prevotella and Gardnerella sp. Antimicrobial metronidazole oral therapy is poorly effective in eradicating the condition and restoring the Lactobacillus microbiota in the vagina. In this study, 40 women diagnosed with BV by discharge, fishy odor, sialidase positive test and Nugent Gram stain scoring, were randomized to receive either two dried capsules containing Lactobacillus rhamnosus GR-1 and Lactobacillus reuteri RC-14 each night for 5 days, or 0.75% metronidazole gel, applied vaginally twice a day . Follow-up at day 6, 15 and 30 showed cure of BV in significantly more probiotic treated subjects compared to metronidazole treatment . This is the first report of an effective cure of BV using probiotic lactobacilli. Given the correlation between BV and HIV, and the high risk of the latter in Nigeria, intravaginal use of lactobacilli could provide women with a self-use therapy, similar to over-the-counter anti-yeast medication, for treatment of urogenital infections.
