VSL#3 and ulcerative colitis. The study demonstrated that VSL#3 is effective in achieving clinical responses and remissions in patients with mild-to moderately active UC4.

Some studies have also been carried out into Crohn’s disease and microscopic colitis.

Symprove and ulcerative colitis. The study found that 76% of those with ulcerative colitis taking part in the study had significant reductions in faecal levels of calprotectin5.

    The DSS-induced zebrafish acute colitis model has a dose-effect relationship for different mortality levels and intestinal inflammation . A high concentration of 0.6% DSS was first added to record the survival of larvae because acute mortality is more straightforward to observe than live imaging . The 0.6% DSS dose results in 62.5% mortality in larvae after 3days of exposure . Prednisolone, the commercial drug for the treatment of IBD, reduces the mortality of larvae. The first anti-inflammatory test identified eight hits with higher survival and causing no malformation of larvae compared to the negative control, namely, L. rhamnosus GG, B. smithii DSM4216, B. smithii XY1, L. parafarraginis XYRR2, L. rhamnosus NBRC3425, B.s coagulans NBRC 12583, B. coagulans XY2, and B. licheniforms XYT3. Other strains may have side effects like edema or severe toxicity, causing greater mortality. Among them, two strains independently caused malformations. One strain increased mortality. Furthermore, six strains caused both side effects .

    How To Take Probiotics

    When you take probiotics for UC, there are two key things to know.

  • You’ll likely need to take them for a while. Your stomach acid kills bacteria, whether good or bad. Experts suggest that you take the probiotics for at least 7 to 10 days to get enough good bacteria. Studies have treated patients with probiotics for up to 6 to 8 weeks. You need millions and millions — possibly billions — to start relieving UC symptoms.
  • To keep up the benefits, you have to keep taking them. If you stop, the balance of bacteria in your colon will change and a flare-up may occur. Talk to your doctor about a long-term probiotic plan.

  • Taking them by mouth may not be best. Although it may not seem pleasant, there may be some advantages to taking probiotics rectally. A smaller dose may be possible because the probiotics skip the stomach acid. This lets more of the good bacteria reach the intestines.     Research into whether probiotics may help people with Crohnâs disease is less clear. The studies have been small, and we need more research into what types of probiotics might work. Because of this, doctors aren’t likely to recommend probiotics to people with Crohnâs disease.

    The researchers screened more than 20 different probiotic species for their ability to tighten the TJ barrier.

    They tested the bacteria in cultures of a human cell line called Caco-2, which scientists often use to model the intestinal epithelium.

    Only LA1 caused a rapid, strong tightening of the TJ barrier.

    The bacteria appeared to achieve this by activating a protein in the membrane of the cells, called a Toll-like receptor.

    To test whether LA1 would have the same effect in a live animal, the researchers administered it orally to mice.

    LA1 rapidly enhanced the TJ barrier in the animals, which then protected them against chemically induced colitis.

    The bacterium also promoted healing of the intestinal barrier in mice with colitis.

    In mice, the same Toll-like receptors mediated these enhancements of the TJ barrier.

    The answer is complicated, according to Berkeley Limketkai, MD, PhD, an associate professor of medicine at UCLA and the director of clinical research at the schools Center for Inflammatory Bowel Diseases.

    Unfortunately, probiotics have not been shown to be helpful for Crohns disease directly, for either induction of remission or maintenance of remission, he says, partially due to a lack of robust studies. A review co-authored by Dr. Limketkai published in July 2020 in The Cochrane Database of Systematic Reviews turned up only two studies that investigated probiotics for the remission of Crohns, and after six months, the treatment showed no difference compared with placebo. A previous review, published in the journal Clinical and Experimental Gastroenterology, included 14 studies on Crohns and 21 studies on ulcerative colitis it found that while probiotics were a beneficial addition to conventional treatment for ulcerative colitis, no such benefit was found for patients with Crohns compared with a placebo.

    For that reason, Limketkai doesnt typically recommend probiotics for his patients with Crohns disease.

    That doesnt mean probiotics arent helpful for Crohns just that enough research hasnt been done yet to prove or disprove their efficacy.

This means that once remission had been achieved less of the probiotic group re-developed symptoms. Whilst this particular strain of E. coli has shown probiotic potential, we would suggest practitioners wait for more research before seeking out any E. coli supplements, as of course other strains of this bacteria are well documented and widely known for their pathogenic effect on the human body. E. coli does not appear on the European Food Safety Authorityâs Qualified Presumption of Safety list.

    The fact that the relapse rate was significantly lower in patients treated with the drug in combination with S. boulardii, suggests a protective effect from the probiotic.
