The Benefits Of Probiotics

Bacteria tend to get a bad rap. When we think of bacteria, we likely think of an infection. However, not all bacteria are bad.

Many bacteria have evolved along with humans in a mutually beneficial relationship, such as the bacteria living in your digestive system. Scientists are beginning to understand how complex these partnerships are between healthy humans and the bacteria inside their guts.

In a healthy gut, theres a delicate balance of different bacteria. Healthy gut bacteria, called intestinal microflora, play an important role in immunity and digestion. Probiotics contain many of the same or similar microorganisms that naturally live in human bodies, which means that, in theory, they can also help with immunity and digestion.

While research around the benefits of probiotics is still relatively new, probiotics are typically considered helpful, because:

  • they may help maintain or improve the beneficial microorganisms in your gut
  • they may positively influence your immune system

Sulfa Drugs Nsaids Who Needs Em Ive Got Yoghurt For My Ibd

For those of you amused by the absurdity of such a notion, there may be more to this statement than you might think. With the rapidly growing threat of antibiotic-resistant bacteria, medical researchers have been directing their attention to probiotic therapy as a means of getting the upper hand on the antibiotic dilemma. Probiotic therapy is a relatively new frontier of investigation whereby you ingest living, non-pathogenic bacteria in order to prevent or relieve a variety of human ailments. There is a wide array of probiotic products being marketed as treatments for gastrointestinal diseases including specially formulated yoghurts. However, contrary to their claims, studies have demonstrated that many of these products do not have the capacity to exert a significant therapeutic benefit.

Safety is just one of the characteristics of probiotic selection. Other traits that an ideal probiotic would possess in order to better perform their therapeutic role include:

  • the ability to survive the harsh travel conditions of the upper GI tract on the way to their site of action,
  • the ability to adhere to the epithelial walls of the intestine and colonize,
  • the ability to compete with pathogenic microorganisms for nutrients and colonization sites,
  • the ability to secrete antibiotic-like substances called bacteriocins, and
  • the ability to provide nutritional services via the synthesis of vitamins1.

Bacillus Smithii Xy1 Effects On Disturbed Innate Immune Genes Expression

Expression levels of the inflammatory cytokine gene IL-1 and TNF- in the DSS group were significantly higher than in the negative control. The expression of the two genes was not significantly different in B. smithiii XY1, LGG, and prednisolone intervention compared with the control . IL-6 gene expression was not significantly different in the treatment groups and the control group. NOD2 is a vital gene that is closely related to IBD pathogenesis. The NOD2 gene was highly expressed in the DSS group but similarly expressed in B. smithiii XY1, LGG, and prednisolone intervention groups. Though the TNF- and NOD2 gene expression of the B. smithiii XY1 group was not significantly different from and LGG group, the mean of the LGG group is higher than the mean of the B. smithii XY1 group, which may relate to the result of the difference in Figure 4C.

Clinical Trials For Probiotics And Ulcerative Colitis

The same strain of non-pathogenic E.coli as was trialled in Crohns Disease has also been extensively trialled in Ulcerative Colitis. This strain of bacteria has been compared with an anti-inflammatory drug used to treat IBD in three separate studies of UC patients. Perhaps the most conclusive of them, by Kruis et al7, showed that out of the 327 study participants, that were treated with either E.Coli Nissle1917 or 500mg of the drug three times daily, the relapse rates were similar in both groups, indicating that E.ColiNissle1917 may be as effective as anti-inflammatory drugs in preventing relapse in UC patients.

A multi-strain probiotic formulation has also been studied for UC. The combination product that was researched, contained three strains of Bifidobacteria, four of Lactobacilli and one of Streptococcus salivarius sp. thermophilus. In this study by Venturi et al8 twenty patients with inactive Ulcerative Colitis were given the probiotic formulation daily for a twelve month period, during which time remission was maintained in 15 out of the 20 study participants. Whilst a 75% success rate appears significant, this trial was not placebo-controlled, so it is difficult to draw any true conclusions.

Probiotics For Inflammatory Bowel Syndrome

Role of probiotics in Inflammatory Bowel disease

Our understanding of Inflammatory Bowel Syndrome is evolving, but we do know that IBS shares a lot of pathological similarities with IBD. Like IBD, IBS involves mucosal inflammation, exaggerated immune response, and probiotic imbalance. You can learn more about IBS in our article, An overview of Inflammatory Bowel Syndrome .

As youll find in the article above, the main types are IBS with diarrhea and IBS with constipation as its predominant features. While there is tons of evidence that probiotics improve IBS symptoms in general irrespective to the type here well only focus on research studying diarrhea or constipation-predominant IBS.

Probiotics for IBS with diarrhea

In one single-blinded RCT, researchers used a cocktail of B. lactis CUL34, B. bifidum CUL20, and Streptococcus thermophiles in a dose of 1 × 108 CFU for four weeks and compared it with a placebo. At the end of four weeks, the probiotic group showed a significant reduction in intestinal permeability scores and an improvement in the clinical symptoms of bloating, flatulence, and abdominal pain, whereas the placebo group did not show much difference from the baseline.29

In another double-blinded randomized control trial, researchers used Lactobacillus plantarum 299v in a dose of 1 × 1010 CFU for 4 weeks and compared its efficacy to a placebo. Results showed the probiotic group had a significant reduction in the frequency of their bowel movements and a reduction in abdominal pain.30

Buyer Beware For Probiotics Users

In the U.S., probiotics are marketed as dietary supplements, so they havent gone through the same rigorous FDA testing as drugs.

This means we have no way of knowing for certain if a product has the bacterial strains or the concentration of bacteria that its label claims.

The higher-ups at the FDA are hoping to change this soon, but for now, we have to go by our own experience and hope for the best.

Probiotic Hydrogels Heal Gut Wounds That Other Treatments Cant Reach

Could open the door to new bioactive healing strategies

In their new study, the team further built on these findings by introducing the machinery for producing one of the mucoadhesive hydrogels based on TFF3 into an E. coli Nissle strain that is a normal gut bacterium that can thrive in the colon and cecum sections of the intestinal tract affected by IBD, and is currently sold in many commercial probiotic formulations. We found that the newly engineered Nissle bacteria, when given orally, also populated and resided in the intestinal tract, and that their curli fibers integrated with the intestinal mucus layer, said first author Pichet Praveschotinunt, who is a graduate student mentored by Joshi.

When we induced colitis in the colons of mice by orally administering the chemical dextran sodium sulfate, animals that had received the PATCH-generating E. coli Nissle strain by daily rectal administration starting three days prior to chemical treatment had significantly faster healing and lower inflammatory responses, which caused them to lose much less weight and recover faster compared to control animals, said Praveschotinunt. Their colon epithelial mucosa displayed a more normal morphology and lower numbers of infiltrating immune cells.

Additional authors on the study are Wyss Institute researchers Ilia Gelfat, Franziska Bahl, and David B. Chou.

Each product at least matched the number of claimed bacteria on the packet and the actual number of bacteria in the product.

Probiotics For Leaky Gut

Best probiotic for dogs with IBD

Leaky gut is when intestinal inflammation causes small gaps to open up in your intestinal wall, which allows bacterial and food fragments to enter your bloodstream. This leakage can lead to uncomfortable symptoms, including food sensitivities, joint pain, brain fog, and digestive symptoms.

Because probiotics help repair the gut lining and restore the gut environment, they help repair leaky gut.

  • One small clinical trial found that probiotics helped to promote intestinal repair in humans .
  • Another trial found positive effects of probiotics when used to treat intestinal impairment after a GI infection [11

Including probiotics may be worthwhile to help repair theintestinal lining damage that occurs with a gut infection or other inflammationyou are experiencing.

A Look At The Research So Far: Clinical Trials For Probiotics And Crohns Disease

A non-pathogenic strain of E.coli has been one of the most clinically trialled probiotics with regard to Crohns disease. In a 1997 double-blind study by Malchow5 E.Coli Nissle 1917 or placebo was taken daily for twelve months by 32 patients with active Crohn’s disease. Both groups also received standard steroid treatment at the same time. The results showed that whilst remission rates were similar for both groups the subsequent relapse rates were lower in the E.coli treated group . This means that once remission had been achieved less of the probiotic group re-developed symptoms. Whilst this particular strain of E. coli has shown probiotic potential, we would suggest practitioners wait for more research before seeking out any E. coli supplements, as of course other strains of this bacteria are well documented and widely known for their pathogenic effect on the human body. E. coli does not appear on the European Food Safety Authority’s Qualified Presumption of Safety list.

The fact that the relapse rate was significantly lower in patients treated with the drug in combination with S. boulardii, suggests a protective effect from the probiotic.

What Is The Research Into Vsl#3 And Ibd

VSL#3 used to be licensed to be available to be prescribed in the UK by the NHS for pouchitis following several clinical studies including one which showed that high doses of the probiotic VSL#3 are effective in the treatment of mild pouchitis1. However, it is now no longer available for prescription in the UK following a review2 which said there was ‘low quality evidence’ of the benefits. Concerns were raised about the size of the studies and that ‘the evidence did not sufficiently demonstrate that the products are clinically effective’.

Studies done into VSL#3 and IBD include:

Ulcerative colitis

There have also been several studies into VSL#3 and ulcerative colitis. The most recent study demonstrated that VSL#3 is effective in achieving clinical responses and remissions in patients with mild-to moderately active UC3.

Crohns disease

Studies into Crohns disease and VSL#3 have focused on:

Paediatric 4, 5 – A study looking at children in remission concluded that VSL#3 didnt influence relapse rate but there was a trend to decreasing faecal markers of gut inflammation4 Another study looking at children with active Crohns concluded VSL#3 has a significant beneficial effect upon disease activity5.

Post-surgery6 – the most recent study concluded that more research into VSL#3 and post-surgery Crohns disease should be carried out.

Ho Do Probiotics For Ibd Help

With probiotics for IBD, sometimes the microbes themselves are most helpful, and sometimes the products they produce can be helpful, separately or additionally. The products they produce, called metabolites, are captured in the laboratory in a growth medium called the supernatant. The actual cells of the probiotics are filtered out of the growth medium, leaving the cell-free supernatant. Since the supernatant is an important part of probiotic action, studying it by itself is another way to know how probiotics for IBD are helpful. Additionally, since there is a layer of mucus that normally separates bacteria from the intestinal wall, and since the supernatant can filter through the mucous, studying the effects of the supernatant, and not the bacteria, provides important insights. When you take probiotics, you may benefit from the activities and cell characteristics of the microbes themselves. Some of these benefits are:

  • Production of short-chain fatty acids
  • Nutrient metabolism and absorption
  • Endocrine-system signaling
  • Blockage, inhibition, displacement or killing of pathogens

You may also benefit from the substances the probiotics produce when the supernatant is produced within you .

Preparing Your Body For Probiotics

Ulcerative Colitis, IBD And The Best PROBIOTICS You Need

Gently cleansing the digestive tract with a safe, all-natural supplement prior to beginning a probiotic regimen is a great way to ensure you get the most from your treatment plan. This helps to create an environment in which probiotic bacteria are able to thrive and helps rid the body of harmful bacteria which may compete with the newly introduced probiotics.

Regular colon cleansing is a great way to help maintain your digestive health but be aware that not all cleansing techniques are created equal. Some can do more harm than good. Do the proper research and know what exactly is in each colon cleansing product.

Is There Harm In Taking A Probiotic

People who live with IBD might wonder what harm there could be in taking a probiotic. For one thing, it might be expensive. For another, it might not help.

While in some cases, probiotics might not have any adverse side effects, for some people it could lead to bloating, gas, or diarrhea. Sometimes these effects will go away over time and with continued use of the probiotic.

Another potential harm is the cost: in some cases probiotics can be quite expensive. If theyre not giving any benefit, that is a problem, especially for people who already have high medical costs.

Whats less well understood is if a probiotic might be used instead of receiving a treatment for IBD that has been proven to work in clinical trials. The harm would be in taking probiotics that might not have an effect and in forgoing other treatment.

Gut Dysbiosis And Its Relation To Ibd And Ibs

The balance of the intestinal microbiota is beneficial for human health. An imbalance in the ratio between good and bacteria gives rise to a condition called gut dysbiosis that can significantly alter your health.

Dysbiosis can be due to three main reasons:1213

  • Unchecked growth of the harmful bacteria.
  • Excessive loss of good bacteria.
  • A lack in the diversity of the gut microbiota.
  • There are several risk factors that can make you prone to dysbiosis. Some of these factors are as follows:1415

The research suggests that dysbiosis plays a pivotal role in the pathogenesis of IBD and IBS.

The three reasons that lead to dysbiosis could exist simultaneously in IBD and IBS. The growth of probiotics belonging to the phylum Firmicutes is globally reduced in IBD and IBS. The population of pathogenic subgroups, like the phylum Proteobacteria, is higher among IBD and IBS sufferers. Finally, in IBD and IBS, the gut loses the microbial diversity as well.1617
