Clinical Trials For Probiotics And Ulcerative Colitis
The same strain of non-pathogenic E.coli as was trialled in Crohns Disease has also been extensively trialled in Ulcerative Colitis. This strain of bacteria has been compared with an anti-inflammatory drug used to treat IBD in three separate studies of UC patients. Perhaps the most conclusive of them, by Kruis et al7, showed that out of the 327 study participants, that were treated with either E.Coli Nissle1917 or 500mg of the drug three times daily, the relapse rates were similar in both groups, indicating that E.Coli Nissle1917 may be as effective as anti-inflammatory drugs in preventing relapse in UC patients.
A multi-strain probiotic formulation has also been studied for UC. The combination product that was researched, contained three strains of Bifidobacteria, four of Lactobacilli and one of Streptococcus salivarius sp. thermophilus. In this study by Venturi et al8 twenty patients with inactive Ulcerative Colitis were given the probiotic formulation daily for a twelve month period, during which time remission was maintained in 15 out of the 20 study participants. Whilst a 75% success rate appears significant, this trial was not placebo-controlled, so it is difficult to draw any true conclusions.

Effect Of Probiotics For Maintaining Therapy In Ulcerative Colitis Crohns Disease And Pouchitis
Clinical Relapse in Maintaining Therapy in Ulcerative Colitis, Crohns Disease, and Pouchitis
To date, maintaining remission is still one of the goals in IBD therapy. Therefore, a total of 16 trials,,,,,,,, that reported clinical relapse rates were combined in the analysis of maintaining remission. Data were available for 1208 patients, of whom 637 received probiotics as maintaining treatment and 571 received controls with the follow-up ranged from 2 to 42 months. Significant benefit in favor of probiotics administration for lower relapse rates was indicated in the meta-analysis . However, there was moderate to high heterogeneity among these studies .
The forest plot of the clinical relapse rates for probiotics compared with control groups in maintaining remission of IBD.
Subanalyses for Clinical Relapse in Maintaining Therapy in Ulcerative Colitis, Crohns Disease, and Pouchitis
Pooled estimates were, however, characterized by considerable heterogeneity in the meta-analysis of overall clinical relapse rates . Subgroup analyses were carried out on the criteria specified in the protocol to attempt to explain and reduce such heterogeneity.
Endoscopic Relapse in Maintaining Therapy in Crohns Disease
Only 3 trials reported endoscopic relapse rates.,, Fortunately, all of the participants pooled in these 3 studies were patients with CD . No statistically significant advantage was found for patients using probiotics for maintaining treatment .
Vsl#3 Probiotic: Ulcerative Colitis And Ibs
VSL#3 is one of the strongest probiotics available at 450 billion bacteria per packet. To put this in perspective, Align, a probiotic promoted to help Irritable Bowel Syndrome , has only 1 billion bacteria per dose. The high concentration of good bacteria in VSL#3 may allow it to positively impact more serious and chronic conditions than other probiotics can.
Probiotics are good bacteria. Good bacteria have a number of purposes in the body. One of their big jobs is to reduce inflammation, a hyperactive immune response associated with allergies as well as with more serious illnesses like ulcerative colitis. At the same time, good bacteria prime your immune system to attack viruses. Probiotics seem to be associated with an all around reduction in bad immunity with a simultaneous increase in good immunity.
Reduced numbers of these friendly helpers may contribute to weakened immunity and allergies, as well as inflammatory GI tract illnesses like ulcerative colitis and irritable bowel syndrome.
Research with VSL#3 over the the last decade has shown that replenishing the gut with a high dose of good bacteria can often put people with inflammatory bowel disease into remission and keep them in remission. Researchers have also found a benefit for IBS with VSL#3

How Probiotics Can Help With Ulcerative Colitis
UC treatment requires a two-pronged approach: management of the symptoms you feel and management of the inflammation in your colon to put the disease into clinical remission. Feeling well and being healed inside dont always go together, Dr. Hudesman says. Until we know more, dont expect that using a probiotic will fix the underlying inflammation.
But if you have noticeable symptoms such as gas, bloating, and loose bowel movements, probiotics might help you find some relief. Foods that bring probiotics into your diet include:
- Yogurt that advertises live, active cultures on the label
- Kefir, a fermented milk drink
- Kimchi, a Korean side dish of salted and fermented vegetables
If youre interested in taking a probiotic supplement, talk to your doctor first. Whether in food or supplement form, probiotics should be used only to complement other treatments for UC. If your doctor has prescribed medication for your UC, dont stop taking it.
Use probiotics with your usual medical therapy, if needed, Hudesman says.
Certain Probiotic Strains Can Help Keep Your Heart Healthy
Probiotics may help keep your heart healthy by lowering LDL cholesterol and blood pressure.
Certain lactic acid-producing bacteria may reduce cholesterol by breaking down bile in the gut .
Bile, a naturally occurring fluid mostly made of cholesterol, helps digestion.
A review of 5 studies found that eating a probiotic yogurt for 28 weeks reduced total cholesterol by 4% and LDL cholesterol by 5% .
Another study conducted over 6 months found no changes in total or LDL cholesterol. However, the researchers did find a small increase in HDL cholesterol .
Consuming probiotics may also lower blood pressure. A review of 9 studies found that probiotic supplements reduce blood pressure, but only modestly .
In order to experience any benefits related to blood pressure, supplementation had to exceed 8 weeks and 10 million colony-forming units daily .
Probiotics may help protect the heart by reducing bad LDL cholesterol levels and modestly lowering blood pressure.

Can Probiotics Help Stop Flare
The answer to this question is likely no. A 2007 review of clinical studies on the use of probiotics for UC flare-ups found that probiotics do not shorten the duration of a flare-up when added to regular treatment.
However, the people in the studies taking probiotics reported fewer symptoms during the flare-up, and these symptoms were less severe. In other words, while probiotics did not end the flare-up faster, they seemed to make the symptoms of the flare-up less frequent and less severe.
Efficacy Of Probiotics In Inducing Remission In Active Cd

How To Take Probiotics
When you take probiotics for UC, there are two key things to know.
To keep up the benefits, you have to keep taking the probiotics. If you stop, the balance of bacteria in your colon will change and you may get a flare-up. Talk to your doctor about a long-term probiotic plan.
Which Probiotics Are Best For Ibd
Before we start to look at the evidence behind the use of probiotics in Inflammatory Bowel Disease , it is important to understand the two different types of IBD, and their clinical presentations. Both Crohns Disease and Ulcerative Colitis share some similarities in symptoms, such as diarrhoea, blood* or mucus in the stools and inflammation and damage to the intestinal wall. However, the defining difference between the two conditions is the location of the ulceration and/or tissue damage. Typically, ulceration in Ulcerative Colitis is limited to the colon, whereas in Crohns Disease the damage can occur at any point in the GI tract, from the mouth to the anus. Crohns Disease is therefore divided in to 5 sub-types, dependent on the exact location in the GI tract that the damage is located.
Ileocolitis: affects the ileum and the colon
Ileitis: solely affects the ileum
Gastroduodenal Crohns disease: affects the stomach and duodenum
Jejunoileitis: affects the jejunum
Crohns colitis: affects only the colon
The different site of damage in the various forms of IBD is important when considering probiotic administration, as different bacteria live in different areas of the intestinal tract, and have different mechanisms of action. For this reason, most clinical trials looking at the potential for probiotic supplementation in Inflammatory Bowel Disease, focus on either Ulcerative Colitis or Crohns Disease, rather than grouping them both together.
