The same strain of non-pathogenic E.coli as was trialled in Crohns Disease has also been extensively trialled in Ulcerative Colitis. This strain of bacteria has been compared with an anti-inflammatory drug used to treat IBD in three separate studies of UC patients. Perhaps the most conclusive of them, by Kruis et al7, showed that out of the 327 study participants, that were treated with either E.Coli Nissle1917 or 500mg of the drug three times daily, the relapse rates were similar in both groups, indicating that E.Coli Nissle1917 may be as effective as anti-inflammatory drugs in preventing relapse in UC patients.

A multi-strain probiotic formulation has also been studied for UC. The combination product that was researched, contained three strains of Bifidobacteria, four of Lactobacilli and one of Streptococcus salivarius sp. thermophilus. In this study by Venturi et al8 twenty patients with inactive Ulcerative Colitis were given the probiotic formulation daily for a twelve month period, during which time remission was maintained in 15 out of the 20 study participants. Whilst a 75% success rate appears significant, this trial was not placebo-controlled, so it is difficult to draw any true conclusions.

    To date, maintaining remission is still one of the goals in IBD therapy. Therefore, a total of 16 trials,,,,,,,, that reported clinical relapse rates were combined in the analysis of maintaining remission. Data were available for 1208 patients, of whom 637 received probiotics as maintaining treatment and 571 received controls with the follow-up ranged from 2 to 42 months. Significant benefit in favor of probiotics administration for lower relapse rates was indicated in the meta-analysis . However, there was moderate to high heterogeneity among these studies .

    Subanalyses for Clinical Relapse in Maintaining Therapy in Ulcerative Colitis, Crohns Disease, and Pouchitis

    Pooled estimates were, however, characterized by considerable heterogeneity in the meta-analysis of overall clinical relapse rates . Subgroup analyses were carried out on the criteria specified in the protocol to attempt to explain and reduce such heterogeneity.

    Probiotics may help protect the heart by reducing bad LDL cholesterol levels and modestly lowering blood pressure.

    Can Probiotics Help Stop Flare

    The answer to this question is likely no. A 2007 review of clinical studies on the use of probiotics for UC flare-ups found that probiotics do not shorten the duration of a flare-up when added to regular treatment.

    However, the people in the studies taking probiotics reported fewer symptoms during the flare-up, and these symptoms were less severe. In other words, while probiotics did not end the flare-up faster, they seemed to make the symptoms of the flare-up less frequent and less severe.

    In our literature search, we found four trials, totally containing 370 patients, which reported the efficacy of probiotics vs. placebo and/or 5-ASAs on inducing remission of active CD.     How To Take Probiotics

    When you take probiotics for UC, there are two key things to know.

  • You’ll probably need to take them for a while. Your stomach acid kills bacteria, whether good or bad. Experts suggest that you take the probiotics for at least 7 to 10 days to get enough good bacteria. Studies have treated people with probiotics for up to 6 to 8 weeks. You need millions and millions — possibly billions — of good bacteria to start relieving UC symptoms.
  • To keep up the benefits, you have to keep taking the probiotics. If you stop, the balance of bacteria in your colon will change and you may get a flare-up. Talk to your doctor about a long-term probiotic plan.

  • Taking them by mouth may not be best. Although it may not seem pleasant, there may be some advantages to taking probiotics rectally. A smaller dose may be possible because the probiotics skip the stomach acid. This lets more of the good bacteria reach the intestines. But for obvious reasons, most people find it easier to just swallow a pill.

    Before we start to look at the evidence behind the use of probiotics in Inflammatory Bowel Disease , it is important to understand the two different types of IBD, and their clinical presentations. Both Crohns Disease and Ulcerative Colitis share some similarities in symptoms, such as diarrhoea, blood* or mucus in the stools and inflammation and damage to the intestinal wall. However, the defining difference between the two conditions is the location of the ulceration and/or tissue damage. Typically, ulceration in Ulcerative Colitis is limited to the colon, whereas in Crohns Disease the damage can occur at any point in the GI tract, from the mouth to the anus. Crohns Disease is therefore divided in to 5 sub-types, dependent on the exact location in the GI tract that the damage is located.

    Ileitis: solely affects the ileum

    Gastroduodenal Crohns disease: affects the stomach and duodenum

    The different site of damage in the various forms of IBD is important when considering probiotic administration, as different bacteria live in different areas of the intestinal tract, and have different mechanisms of action. For this reason, most clinical trials looking at the potential for probiotic supplementation in Inflammatory Bowel Disease, focus on either Ulcerative Colitis or Crohns Disease, rather than grouping them both together.
