What The Research Says
There are two main types of probiotics. Some studies have suggested that the best type for IBS is bifidobacteria, as long as you take it in large enough amounts. The other main type is called Lactobacillus. Bifidobacteria is the one usually found in supplements,” says Dr. Rizk. “Lactobacillus is the one usually found in yogurt.”
A major review published in June 2018 in the American Journal of Gastroenterology looked at the potential benefits of both kinds of probiotics for diarrhea. The review authors found moderately good evidence for preventing antibiotic-associated diarrhea with use of both bifidobacteria and Lactobacillus.
One study, published in the journal BMC Gastroenterology, which looked specifically at the potential effects of probiotics for IBS, suggested the theory may lean more toward the side of myth. In this study of 76 people with IBS, one-half consumed a dairy product with probiotics twice a day, while the others consumed one without probiotics twice a day. After four weeks, IBS symptoms had improved in 57 percent of the probiotics group and 53 percent of the nonprobiotics group after eight weeks, improvement in IBS symptoms was recorded for 46 percent of the probiotics group and 68 percent of the others leading the researchers to conclude that probiotics were no better than nonprobiotics for IBS.
And a review published in 2016 in the Journal of Human Nutrition and Dietetics found inconclusive evidence that probiotics can treat IBS symptoms.

Probiotics For Inflammatory Bowel Syndrome
Our understanding of Inflammatory Bowel Syndrome is evolving, but we do know that IBS shares a lot of pathological similarities with IBD. In one single-blinded RCT, researchers used a cocktail of B. lactis CUL34, B. bifidum CUL20, and Streptococcus thermophiles in a dose of 1 × 108 CFU for four weeks and compared it with a placebo. At the end of four weeks, the probiotic group showed a significant reduction in intestinal permeability scores and an improvement in the clinical symptoms of bloating, flatulence, and abdominal pain, whereas the placebo group did not show much difference from the baseline.29
In another double-blinded randomized control trial, researchers used Lactobacillus plantarum 299v in a dose of 1 × 1010 CFU for 4 weeks and compared its efficacy to a placebo. Results showed the probiotic group had a significant reduction in the frequency of their bowel movements and a reduction in abdominal pain.30

The first thing to consider about probiotics making your IBS worse, has to do with the food that might be in the probiotic. Many probiotics have ingredients in them that is food for the probiotic bacteria and the bacteria in your microbiome. This ingredient is called a prebiotic and these are good if you have a healthy microbiome. However, if there’s a prebiotic in your probiotic then things could get worse for you, if you have IBS. Specifically, this problem might arise if you have an imbalance in your microbiome called dysbiosis.
Dysbiosis is basically an imbalance in the bacteria in your microbiome and there are different ways this imbalance can manifest. If you have a specific type of dysbiosis called SIBO or small intestinal bacterial overgrowth, each time you eat your probiotic you are feeding the bacteria in the small intestine . This causes those bacteria to grow more, expand, and create a lot of gas. The gas causes discomfort cramping and a lot of the IBS type symptoms.

During Early Stage Sibo Treatment Avoid
In the early stages of SIBO treatment, I suggest you Avoid probiotics that Contains maltodextrin, tapioca starch, dairy, soy, Unfortunately, many of the most highly recommended and prescribed probiotics on the market contain these.
Ive mentioned this in a past video but is it also applicable here,- avoid fermented foods in the early stages due to the possibility of histamine exposure. That means no kombucha, no fermented foods, and being careful with the probiotics that you are taking as some may increase histamine levels.
In the early stages of treatment, its not a bad idea to consider takeing some of the histamine lowering probiotic strains.

Should Probiotics Be Taken On A Low Fodmap Diet
The low FODMAP diet is one of the proven methods for treating IBS.
It involves first eliminating most FODMAPs from the diet for a 3-8 week period so that you are virtually symptom-free. Then you begin to systematically reintroduce them in order to identify the culprits, followed by a modified diet long term.
For this to work, its important that other possible triggers of digestive stress are minimized. That includes caffeine, spicy foods, fatty foods, alcohol, and fiber supplements among other things. Removing these ensures that if symptoms do occur, you will be able to trace it back to an exact FODMAP group.
While probiotics can be highly beneficial, some people experience a worsening of symptoms. Given this risk, its typically not recommended to start a new probiotic supplement during any phase of the low FODMAP diet.
The exception is if you already use a probiotic supplement before starting a low FODMAP diet. In this case you you can continue to do so throughout.
